RESUPPLY PROGRAM TERMS & CONDITIONS

These Terms & Conditions govern participation in Lactation Depot's Breast Pump Parts & Breastfeeding Supplies Resupply Program.
PROGRAM DESCRIPTION
The Resupply Program allows eligible customers to receive replacement breast pump parts, milk storage bags, and other breastfeeding supplies in accordance with insurance coverage, prescription requirements, manufacturer recommendations, and payer guidelines.
ELIGIBILITY
Participation in the Resupply Program does not guarantee eligibility for products.
Eligibility is determined based upon:
• Insurance coverage
• Plan limitations
• Prescription requirements
• Medical necessity requirements
• Product availability
• Manufacturer guidelines
Lactation Depot reserves the right to verify eligibility before each shipment.
NO GUARANTEE OF COVERAGE
Insurance verification is not a guarantee of payment.
Insurance carriers may:
• Deny claims
• Modify benefits
• Change eligibility requirements
• Apply frequency limitations
• Require additional documentation
Coverage decisions remain solely the responsibility of the insurance carrier.
CUSTOMER RESPONSIBILITIES
Participants agree to:
• Provide accurate insurance information
• Notify Lactation Depot of insurance changes
• Notify Lactation Depot of address changes
• Respond to eligibility verification requests
• Confirm product needs when requested
Failure to provide updated information may delay shipments or result in claim denials.
RECURRING COMMUNICATIONS
By enrolling in the Resupply Program, customers authorize Lactation Depot to contact them regarding:
• Eligibility verification
• Product availability
• Prescription renewals
• Shipment reminders
• Insurance updates
Communications may occur by phone, email, SMS, or other electronic means.
PRODUCT SUBSTITUTIONS
If a product becomes unavailable due to manufacturer shortages or inventory limitations, Lactation Depot may offer a substantially similar product of equal or greater value.
SHIPPING
Shipment timing is dependent upon:
• Insurance eligibility
• Prescription validity
• Inventory availability
• Manufacturer supply levels
Estimated shipment dates are not guaranteed.
CANCELLATION OF PARTICIPATION
Customers may opt out of the Resupply Program at any time by contacting Lactation Depot.
Lactation Depot may suspend or terminate participation if:
• Eligibility requirements are not met
• Insurance coverage cannot be verified
• Required documentation is unavailable
• Fraudulent information is suspected
FINANCIAL RESPONSIBILITY
Customers remain financially responsible for:
• Deductibles
• Copayments
• Coinsurance
• Non-covered items
• Denied claims
• Retroactively denied claims
LIMITATION OF LIABILITY
Lactation Depot shall not be liable for delays, denials, coverage determinations, inventory shortages, or manufacturer supply interruptions.
